The game was called Stay Alive. Soul survivor is from the TV ad. A group of kids find the game washed up on the beach and decide to play. At the end of the ad, the winner of the game says "I'm the soul survivor".

JC's right! Stay Alive was a Milton Bradley game from 1971, retailing for three dollars, per the Sears Toy Catalog. The only Soul Survivor game is a modern day role-player with an Area 51 setting. Commercial for Stay Alive aired on the "Real Jerry Lewis" cartoon show (5/1/71) over ABC-TV.
